Abstract:
Paid services of clinical pharmacists are very important for the development of positions of clinical pharmacists in various settings of care and for the development of clinical pharmacy field. Legislative changes in the Czech Republic helped to create fully paid clinical pharmacy positions in acute care and ambulatory care with promises of achieving similar success also in other settings of care. What legislative and professional steps were needed to create such health care positions? What does daily work of clinical pharmacists require in acute and ambulatory care? This symposium can inspire you how to achieve similar successes through the necessary legislative, administrative and professional steps and what everyday work of clinical pharmacists on fully paid clinical pharmacy positions require.

Care for geriatric patients in the Czech Republic and cooperation of clinical pharmacists, pharmacists, geriatricians and general practitioners in various settings of care

Abstract:
Geriatric patients often suffer from multimorbidity, polypharmacy and are usually frequently hospitalized due to drug-related problems. Health status of older adults can be substantially modified by successful compensation of disorders, various interventions (pharmacological, non-pharmacological) and sufficient professional support. Quality care for complex older adults requires interdisciplinary cooperation of various healthcare professionals across different settings of care and substantial is the role of geriatricians (experts in complex geriatric clinical assessments and specific geriatric interventions), clinical pharmacists (experts in complex medication reviews and pharmacotherapy risks/benefits) and other pharmacists providing drug information, medication adherence support, self-monitoring support etc.

Symposium of the ICARE4OLD H2020 project

Abstract:
The I-CARE4OLD Horizon 2020 project was funded by the European Commission with the aim to individualise healthcare interventions for the ever growing group of older persons with chronic complex conditions. From 2021-2025, the international experts in various healthcare disciplines, health statistics and artificial intelligence are combining their clinical insights on specific pharmacological and nonpharmacological strategies and their real outcomes in geriatric patients together with scientific works on big, real world data acquired for over 30 years in older adults in long-term care with the aim to develop a state-of-the art digital platform providing decision support for healthcare professionals treating this particular complex group of persons. The developed solution is expected to lead to improved treatments, enhancing the quality of life of older persons with chronic complex conditions and their relatives, better quality of care, and to reducing costs of care for the society as a whole. Symposium of the EuroAgeism H2020 project

Abstract:
Europe has the largest percentage of older adults worldwide. Yet, there is a high prevalence of ageism; the complex and often negative construction of older age, in society at large, and among researchers and practitioners in particular. In contrast to the wide prevalence of ageism, the majority of research, to date, has targeted racism and sexism and neglected to address ageism. A similar neglect is found on the policy side, as age is still not an explicit part of a U.N. declaration of human rights. To address these shortcomings, EuroAgeism Horizon 2020 project was focused on harmonising different theoretical and methodological disciplines in the field of ageism. It aimed to bring together science and policy by creating an opportunity for a true dialogue that will allow graduates of the program to capture positions as scientists, educators, science advisers, policy advocates, lobbyists, or legislators in the field of ageism. EuroAgeism project advanced the field by delivering an attractive multi-disciplinary (social science, health science, engineering, and design), inter-sectorial (academia, healthcare, non-governmental, inter-governmental, and governmental organizations), international (North, East, and Central Europe, Middle East, Pan-European, and international) research, training and supervision program on ageism in various aspects and fields, including inappropriate prescribing and medication safety (ESR7 project within the EuroAgeism H2020 programme). The research was a direct response to the European Commission's Active and Healthy Ageing initiative, with an overarching goal to improve the quality of life of older people. By pursuing 15 research projects, the project aimed to achieve the following goals: a) explore ways to improve the active participation of older adults in the workforce; b) address ageism in relation to access to goods and services, including appropriate medications, appropriate prescribing and medication safety services, and c) to promote an age-friendly society, which helps older adults to realize their full potential. EuroAgeism was a policy oriented research project dedicated to dissemination of findings also to policy stakeholders (WHO, UNECE, European Centre in Vienna, Age Platform Europe) and to the lay public to decrease ageism in everyday life, in medication use and the clinical and social practice. This Symposium shows an overview of selected works conducted during the project in medication area during ESR7 program of the EuroAgeism H2020 project particularly in Central and Eastern European countries.
