Antidepresiva v těhotenství; materno-fetální transport a vliv na placentární transport a metabolismus serotoninu

Depression affects up to 25% of pregnant women, with 7-13% being exposed to at least one antidepressant drug during pregnancy. Although it is still unclear whether/how antidepressants affect development of the fetus, current guidelines recommend treatment of depressed pregnant women. By inhibiting serotonin reuptake or metabolism, antidepressants crossing the placenta may disturb fetal serotonergic system, leading to impaired neurodevelopment. In addition, serotonin was found to be transported and metabolised within the placenta, therefore, it is feasible to assume that antidepressants may indirectly disrupt placental handling of serotonin. Consequently, the aims of the current project are: 1) to investigate mother-to-fetus passage of selected antidepressants (citalopram, sertraline, paroxetine and venlafaxine) and their interaction with placental efflux transporters and 2) to evaluate possible effect of these drugs on placental serotonin transport and metabolism. A combination of experimental approaches will be utilized, including in situ perfusion of the rat placenta, in vitro studies in human choriocarcinoma cell line BeWo, ex vivo uptake studies in fresh villous fragments isolated from human term placenta and in vivo long-term study of antidepressants use in rats. Results obtained from this project could increase our understanding of interactions between antidepressants and the feto-placental unit and assist in optimizing depression pharmacotherapy during pregnancy.
