Clinical Pharmacy - Risk Management in Pharmacotherapy

The goal of this subject is to minimize the risks of drug related problems in the adverse drug reactions, medication errors and the problem of communication at the physician/pharmacists, physician/burse, health care workers/patients etc. levels.

Syllabus

Lectures and Seminars

  • Adverse drug reactions, basics of risk management of pharmacotherapy, algorithms “SAZE” and “SOUP”
  • How to use clinical pharmacokinetics, pharmacopeidemiology in the risk management
  • Risks of non appropriate dose, drug-drug interactions
  • How to solve the case reports
  • Adverse drug reactions and their management in betablockers, diuretics, ACEI, proton pump inhibitors, amiodarone, antidepressive agents, anti-infectious agents, anticoagulant agents, antidiabetics

Bed Site Teaching

  • Communication with the patient – judging of diagnosis, drug anamnesis
  • The analysis of cases – the synthesis of experience, knowledge and skills of clinical pharmacy, pharmacology and another drug and disease related subjects –to look for relevant information related to judging of the rational pharmacotherapy
  • Target discussion on the patients suffering from arterial hypertension, diabetes mellitus. Polypragmasia
